Equipment Loan Checkout Agreement
Borrowers checking out equipment from Classroom Services agree to the following terms:
Use of equipment
Valid ID is required for equipment checkouts. Equipment shall be used in a careful manner and as intended for academic pursuits and campus events. Academic / curricular use takes priority to general usage. Equipment is not to be used for revenue-generating activities (freelance work, events where tickets purchased for attending, etc) unless authorization is received in writing from a Classroom Services manager or supervisor.
The borrower agrees to comply with all applicable federal, state and local laws, university policies, the equipment manual, and the terms and conditions relating to the possession, use, or maintenance of the equipment. Borrower agrees to return equipment on time and in the same condition as originally loaned.
Loss and damage
The borrower assumes all responsibility for any damage, loss or theft of equipment and accessories connected to their check out. All costs associated with the loss, theft, or damage of the equipment while on loan shall be the sole obligation of the borrower.
Replacement charges are based on the cost of parts and labor to repair the equipment and consideration of “normal wear and tear,” or the current cost of replacing the unit, as determined by ATUS.
Late fees
Failure to return the equipment at the scheduled return time may result in fines, and/or the loss of future loan privileges as determined by ATUS.
Late fines are calculated based on equipment type. Higher-value items are $12/day and lower-value items are $2/day and will be calculated from due date as determined by ATUS. It is the borrower's responsibility to return equipment on time.
Reservations
Reservations are available for limited items and will be held for only one hour past the reserved time. If equipment is not picked up within that timeframe, it will become available for other requests.
